On 26th September 2013 Adam acted for the Prison Service at the Inquest into the death of Peter Mason, a 58 year old computer IT director.  The Inquest was heard by the Deputy Coroner for Gloucester, Mr Tom Osborne and a Jury.  

Mr Mason was remanded into custody at HMP Gloucester on 5th July 2012, following his arrest for attempted murder of his estranged wife and her new partner.  Following the attack on his wife and her new partner Mr Mason had attempted to commit suicide.  After been arrested Mr Mason was assessed as being at a high risk of committing suicide.  At HMP Gloucester Mr Mason was managed under the Prison Service Assessment, Care in Custody and Teamwork system for four weeks until he his mental state was assessed by the Prison Healthcare team to have stabilised.  He had integrated into Prison life, obtained a job in the library and helped other prisoners with legal letters.  On the early evening of Saturday 19th January 2013 Mr Mason hanged himself in his cell.  Mr Mason left a note (described by a Police Officer as a “goodbye letter”) addressed to his two children.  

The Jury returned a verdict of suicide.
