  • GP surgery for failing to diagnose breast cancer, patient died  
  • GP surgery for failing to make specialist emergency referral leading to loss of sight in one eye
  • hospital midwives/obstetricians for failure to monitor heart rate of baby leading to hypoxia and brain damage (3 cases) 
  • health authority for failing to provide appropriate post-natal literature alerting parents to signs of hip deformity in a child 
  • dental clinic for negligent treatment relating to inserts and bridge hospital for negligent treatment of fistula leading to patient’s death 
  • hospital for failing to replace shunt of cerebral palsy sufferers timeously (2 cases)
  • GP/hospital for failing to diagnose or treat in good time cauda equina syndrome (several cases)
  • Hospital for negligently performed disc repair
